PLPC Waiver

The Prescott Lakes Pickleball Club requires all learners/students who participate in lessons/instruction/mini-camp to sign the Waiver form. You may print a copy here, or obtain a copy at the time of the lesson/instruction/mini-camp.  No instruction will be given without the completed and signed Waiver.
