PLPC APPLICATION / CODE OF CONDUCT
Welcome to the Prescott Lakes Pickleball Club. We are happy you are considering becoming a member. To become a member, YOU MUST SIGN TWO DOCUMENTS!
1. Please sign and print your name and date the Code of Contact.
2. On the PLPC Application, please fill in the contact information including your name, email, and phone number(s).
3. If you wish to order a name tag, check the box and enter your name as you wish it to appear on your name tag.
Please be sure to print both documents (you may print them back to back on one sheet of paper), fill in and sign, and turn them, along with a check made payable to PLPC for your annual dues, plus your name tag if ordered, to any Board Member.
